how to become a patient
To schedule your first appointment, call 775-786-4673, Monday–Friday, 8am-5pm, and ask to schedule a “new patient appointment.” An Intake & Eligibility Specialist will take your information and partner with you to schedule your first appointment.
paying for care
HOPES takes most insurance plans, Medicaid, and Medicare. HOPES also cares for uninsured patients. If you don’t have insurance, we use a sliding fee scale which adjusts cost based on what you can afford to pay. Our Intake and Eligibility team meets with all new patients to make sure they can get the care they need.
What to bring to your first appointment
To ensure a smooth transition of services to HOPES, please bring these items to your first appointment:
ID
We will need some type of identification ranging from ID’s to birth certificates.
Insurance Card
If you have an insurance card, please bring it. If you don’t have insurance, still come in and we can help you get seen.
A list of the medications you are currently taking
If it’s easier, you can bring the bottles in.
Medical records
From either the hospital, your last emergency room visit, or previous physician.

You pay for your health care based on what you can afford.
Language and communication resources
If you don’t speak English or have visual or hearing impairment, HOPES has resources to communicate with you.
