a Legacy of Caring

Since 1997, HOPES has been on the front lines of community health, caring for those with the fewest resources and options, many of whom are living in crisis every day. We tackle our community’s hard problems with resolve and compassion — from chronic disease to addiction. 

From our early days as a single-room HIV/AIDS clinic to the multi-facility community health resource center we are today, HOPES continues to serve northern Nevada’s vulnerable, overlooked, and underserved. 

While patients typically come to HOPES seeking medical care, our team works closely with on-site case managers to meet all patients’ wellness needs, connecting them with meals, clothing, shelter, mental health care, addiction recovery support, and other vital services.

Everyone who walks through our doors is greeted with kindness and treated with respect and compassion. We welcome everyone, wherever they are in life, and provide a safe and accepting place to access comprehensive, patient-centered healthcare services. People experiencing homelessness, those without documentation or health insurance, or those struggling with substance use or mental health are among the groups we welcome and care for with dignity. 

As your medical home, Northern Nevada HOPES provides accessible, continuous, comprehensive, patient-centered, coordinated, compassionate, and culturally effective care.  We are committed to coordinating your treatment through your primary care provider and care management team to ensure you receive the most up-to-date, evidence-based care. Our team offers whole-person care, evidence-based preventive and wellness care for chronic conditions, care when you are sick, and education and self-management support.

Northern Nevada HOPES has achieved Bronze status in the Advocacy Center of Excellence (ACE) Program, and is currently the only ACE health center partner from Nevada.

The ACE program recognizes Community Health Centers that demonstrate excellence in formalizing their advocacy program to make it an institutional part of health center operations. As a Bronze ACE, our health center serves as an example to other organizations striving to achieve advocacy excellence. 

The 2024 Candid Gold Seal of Transparency shows donors that HOPES is a non-profit committed to financial transparency and Diversity, Equity, and Inclusion in our leadership. Northern Nevada HOPES was inducted into the “Best Places to Work in Northern Nevada” Hall of Fame after winning first-place three of five years. The awards program is sponsored by the Northern Nevada Human Resources Association (NNHRA) and recognizes top northern Nevada companies for employee engagement and investment.

HOPES is required to report on a core set of measures each calendar year to be assessed by the Health Resources and Services Administration on their impact and performance. (Note: HOPES operates under the name Northern Nevada HIV Outpatient Program, Education And Services, it’s original name back in 1997)

Designations

501(c)(3) nonprofit

Northern Nevada HOPES is a 501(c)(3) nonprofit organization. As a Community Health Center, HOPES receives funding from federal grants and private donations to provide services. Thanks to the generosity of people like you, HOPES is helping thousands of underserved people in our community.

Federally Qualified Health Center

Northern Nevada HOPES is a Federally Qualified Health Center (FQHC). FQHCs include all organizations receiving grants under Section 330 of the Public Health Service Act (PHS). FQHCs must serve an underserved area or population, offer a sliding fee scale, provide comprehensive services, have an ongoing quality assurance program, and have a governing board of directors. As a health center, HOPES has stringent criteria for all requirements regarding administrative, clinical, and financial operations, put forth by the Health Resources and Services Administration.

Patient Centered Medical Home

Northern Nevada HOPES is a Patient Centered Medical Home (PCMH). The PCMH is a model of care that aims to transform the delivery of comprehensive primary care to children, adolescents, and adults. Through the medical home model, practices seek to improve the quality, effectiveness, and efficiency of the care they deliver while responding to each patient’s unique needs and preferences.

National Health Services Corps

Northern Nevada HOPES is a National Health Service Corps (NHSC) site. NHSC helps bring health care to those who need it most. Since 1972, NHSC has been building healthy communities by connecting primary health care providers to areas of the United States with limited access to care. Today, 9,600 NHSC members provide culturally competent care to more than 10 million people. This care is provided at 5,000 NHSC-approved health care sites in urban, rural, and frontier areas. As an NHSC site, Northern Nevada HOPES does not discriminate in the provision of services to individuals based on their ability to pay, race, color, sex, national origin, disability, religion, or sexual orientation. HOPES offers discounted services to qualifying patients and also accepts insurance, Medicaid, Medicare, and Nevada Check-up.

Federal Tort Claims Act

Utilization Review Accreditation Commission (URAC)

Northern Nevada HOPES’ pharmacy is fully URAC accredited, with special accreditations such as mail service pharmacy and specialty pharmacy. This means we are committed to upholding the highest standards in medication prescription and delivery, and that we adhere to the most rigorous standards of quality for our patients.

URAC is the nation’s largest independent healthcare accreditation organization and the undisputed leader in pharmacy accreditation.
