Manage your patient account

All current HOPES patients can use the patient portal account to send their doctor, or any medical provider, a message; pay a bill; set up appointment reminders; see test results; and join a telehealth visit. After you become a patient, HOPES will provide you with portal login information.

Our 5th Street clinic has a pharmacy located on the first floor for your convenience. You can request a refill for many prescriptions through the patient portal or by phone.

  • Who can access this service?
    • Any existing patient that has an appointment scheduled at HOPES (Any patient under the age of 18 must be accompanied by an adult).  
  • How can patients schedule their own Uber ride for an appointment?
    • Patients can call the HOPES main line and press Ext.6015 to be directed to the scheduling department. Monday through Friday from 8am-5pm.
  • Can patients/staff members request a ride for the same day?
    • No. we ask that staff and/or patients schedule at least one business day ahead of time and no more than 30 days before the scheduled appointment.
