Case managers at Northern Nevada HOPES (HOPES) provide non-medical case management services to our patients. They provide referrals to resources and services that help improve our patients overall health outcomes.
Available Services
Our case managers help patients connect with:
- Safe, affordable housing
- Food security
- Job training
- Financial support for health services
- Enrollment resources for insurance
- Pediatric resources
- HIV+ care resources
- Gender-affirming care (GAC) resources
Our case managers can provide a lot of support for patients, but the following are things they DO NOT connect patients with:
- FMLA
- Workers Comp
- SSI/SSDI application
- Insurance claim disputes
- Document service disputes
- SNF, medical group homes, home health
- Specialist referrals
